ENROLLMENT CLOSED FOR 2012-13
The enrollment & behavior forms are required to be submitted and approved before a student may attend the afterschool program. (see link below)
The Parent Handbook (info sheets) explains the program in detail and includes important information pertaining to the attendance policy, pick-up times/procedures, student safety, and discipline. The handbook can be also be printed for quick reference.
Complete and return the enrollment/behavior form to the Czech Point Afterschool program office located in the Community Center Bldg. or any school office.
Please be aware of the changes in the attendance requirements and pick-up times/procedures for the 2012-13 school year.
