2013-2014 ENROLLMENT IS OPEN
The enrollment & behavior forms are required to be submitted and approved before a student may attend the afterschool program. (see link below) You must receive an acceptance letter before your child can start attending the program.
The Parent Handbook (2 info sheets) explains the program in detail and includes important information pertaining to the attendance policy, pick-up times/procedures, student safety, and discipline.
Complete and return the enrollment & behavior forms to the Czech Point Afterschool program office located in the Community Center Bldg. or any school office. Keep a copy of the parent information for your reference.
Please be aware of the changes for the 2013-14 school year.
